I've seen them broken first hand, as well as seen pictures here, but never snapped one myself.

was it from some abnormal abuse or from regular type use?

My first range trip with my first build. After ~100 rds I had some sort OOB that caused alot of damage.
The bolt carrier (LW Smith Enterprise's unit) wedged onto the hammer and into the receiver extension. The bolt carrier cracked down one side of the ramp and the hammer was twisted. The receiver extension, an MiC CAR stock setup from ebay(way back when), split through the receiver threads between the receiver and the endplate. I replaced everything with Bushmaster parts.
