For the yes or no questions, if you answer no to one you have to "attach documentation explaining your answer". What kind of documentation we talking here? Official or just a couple sentences saying what happened?

Uh, more specifics might be helpful here. Which question do you need to provide more info? As I view the application, it seems unlikely that you'll be approved for a permit if you answer yes to any of those big 10 questions. The "arrested for domestic violence" would probably need some sort of court documentation that you weren't charged or were acquitted. Similarly, an expunged felony would still be discovered. If you were convicted of some white collar crime 20 years ago, got it expunged and have behaved, you might be the permit. If it was a violent crime felony, you'd have to have a really good explanation.
Now, I'm not a lawyer or a state employee, so those are just my guesses. But I've had a Utah CCW since the shall-issue law was passed in the 90s.
