Just got finished with a guy who was beaten up by some of his colleagues at the county jail. He had some facial lacerations that needed repair, and probably a blowout fracture of his right orbit. He has liver disease, so he bled a fair amount. His head CT showed a right sided subdural hematoma (blood between the brain and skull), partially compressing his brain on that side. The question is, will this bleeding stop on its own, or since he has cirrhosis, which can compromise your ability to clot your blood, will it get worse and require that he get a hole drilled in his skull. Moral of the story--stay out of jail.

If the bleed is small it may stop on it's own and the fluid will get reabsorbed back into the vasculature. A burr hole is not usually effective for acute Subdural hematomas. If the bleed starts to compromise his neurological status the doctors will more than likely perform a craniotomy.
