I like the way NPR does it in their news. If the story involves the president, the FIRST time they refer to him, it's "President" Obama, and subsequent times in the story, he is referred to as "Mr" Obama. (I believe they've been doing that for a long time, so it's not a new thing).

Yes, but most media organizations don't follow it. NPR is trying to follow it, despite their liberal leanings (they know which party in Congress is keeping them funded). The WSJ does it. But that's about it... the editorial staff has tilted the playing field.
