I've been doing a lot of reading of Tom Clancy novels lately and am currently about 3/4 through Red Storm Rising. Lots of naval warfare in the story with many mentions of the mk-48 adcap torpedo..

Sooo, did a little web search and found this:

Humm.... if it was an old ship... it might have been the boiler going.... ya never know if they brought it out on it's own power....

This is how most of the better anti-surface ship torpedoes work.  They actually explode under the ship and blow the water away from the keel.  The resulting strain, combines with the explsoive damage, breaks her back at the detonation site.

Also correct.

The Mk48 is a viscous little sucker. Especially the ADCAP (Advance Capability) version, which was designed to punch through the hull of a Typhoon class SSBN.

I definitely would NOT want to receive THIS type of football....

Remember a few years ago there was an abandoned ship off the Oregon coast?  The Navy tried burning the fuel on board, but that didn't work out.  Plan B was to tow it to deep water and torpedo it, but the tow line broke. The ship washed up on the shore, and the CG detonated a bunch of C4 to burn the oil.  It was the ship that would not die.
