What if the guy trying to administer the execution said "Fuck it", grabbed a fire extinguisher off the wall and smashed the guy's head in? What legal trouble would he be in? |

As others have said, there's always IO, femoral, jugular, etc. Where there's a will and a needle large enough (and sometimes a running start), there's vascular access. |

the five executioners were equipped with .30-30 caliber rifles and off-the-shelf Winchester 150 grain (9.7 g) SilverTip ammunition. The condemned was restrained and hooded, and the shots were fired at a distance of 20 feet (6 m), aiming at the chest

Maybe we should bring back the fireing squad. At least that worked the first time, every time. Yup, sounds like a plan to me, we could line up about 10 death row prisoners at a time, saves money that way.
