In the Army, it's issued in boot camp and mandatory for wear.

Later in your career, it's optional, but most will tell you (especially those of us who failed to wear it) that they wish they had.

Certainly, in many cases it's incredibly impractical. But for stuff where you KNOW you're going to need it (ranges, artillery fire, working on heavy machinery, etc) it's a great idea.

it is mandated!

downrange i wore my truck comms but the times i forgot it ie AT4 firing, mortars, gunfire etc i really wish i had it. as an NCO now i always have some. foam ones pinned into my patrol cover, two or three extra sets in my assault bag, my surefires in my shoulder pocket and another set in my general purpose kit on my armor. youll never regret having them so..........

were issued the "combat ear plugs" that have a little valve in them to let some conversation in but keep heavy sounds out- wore every time we went out the wire- also an option is something like peltor tactical 6 muffs- very low profile and if you get the behind the neck band model fit under a helmet nicely.
some guys will talk about and I have experienced auditory exclusion in a gun fight- meaning nothing sounds loud during the fight- but after you will still have damaged hearing

I'd say it really depends on your job. It would be hard to hear anything if you are wearing ear pro as infantry or a scout. I'd rather have some ear damage than a sucking chest wound. I don't know any 11B's that wear them on live missions.
