This guy left his in somehow after cleaning and had a massive kaboom.
He was shooting a .17 VHA.

A cleaning jag.  You know, those things you screw onto the end of a cleaning rod to clean your weapons with.  You can see it sticking out of the end of the barrel in his picture.

People don't do the thumbnail in the breech and sight down the barrel to check cleanliness anymore?

Sounds like it happened at the action. "Jag was still in the barrel, loaded a round and pulled the trigger, over pressure opened up the action and the brass hit him in the face."

I know, never say never, but my cleaning routine consists of using compressed air to do a final clean/dry of the barrel, then a visual of the rifling. I'm kinda OCD about it.
