Ok, so I'm performing an AK open circular amputation. The skin and muscle have already been cut and retracted. I sutured all the major bleeders but when I took the tourniquet off one started squirting again. I tried suturing it again but it's kind of small and I'm having a tough time. Can I just take a soldering iron to it? Maybe just make a slipknot with the thread and squeeze it off that way?

Also, I started cutting on the femur (with tourniquet still on because of that pesky blood vessel) with a fine tooth saw and it's getting a lot hotter than I expected. Will it get hot enough to damage the femur? Any tips? Also the edge of the cut bone is really rough, can I file it round?

Also what are your thoughts on traction methods?

Tie it off with suture, it won't get too hot, filing is ok.

I find that the inexpensive wood-burning kit from wally world works better than a soldering iron as an improvised Bovie.

Orbital sander will put a nice clean, round surface there. You don't want a rough surface.

Be sure to leave about 5" of skin overhang to sew the end up.

Ok just got done.

Ended up using the soldering iron and it worked OK.

Irrigated while sawing kept the heat down. Also filed the sharp edges off.

Guy is still out (maybe a lil too much anesthesia?) and I rigged up some traction using plain medical tape and some weights dangling off the bed.

This is still a ways down the road, but what are your thoughts on early flap closure?

BTW, this is on a crusty old guy who lives nearby. He jacked his left leg up on a bush-hog then let it get gangrene. Normally I'd tell someone to just go to the hospital but this is a guy who would literally rather die than go to the hospital (think he had a child die years ago). He's trading me a 1903 springfield and 400 rounds of very vintage ammo for my services.

Go to a dentist to get pain pills and antibiotics. A dentist will never suspect a home amputation, thus avoiding pesky questions.
