I'm trying to fill that out and it won't let me type in the fields.  I saved it to my computer and it still won't... and when I look up the security settings, it says that Document Assembly is not allowed.

I'm messing with it and I can't figure out how to allow document assembly.  I would think there is a way if its saved on my own computer.

Have you tried using the seventh icon from the left?  It's just to the right of the little "hand" icon on your menu bar.  By clicking on that it will change your cursor from the little hand icon to an icon that looks like a capital "I" that allows you to type in your info.  You probably won't be able to save it without Acrobat Pro, but it will still be able to be printed.

That will put the cursor on the page (flashing like when your typing a reply here) but when you try to type nothing goes on the document.

You cant use Acrobat Reader to edit a PDF. That file was intended to be printed out and filled in by hand.

You're right - there's some kind of problem with the form.  

How about Plan "B".  Copy the entire form, paste it into MS Word and then fill it out.  Then format it to look like the original form.

There are no boxes to fill out since the form was not designed with any.

Print it and fill it out.
