Either use Ask The Experts or send an email to [email protected], with a subject of "EForms DRAFT watermark". Include control number, serial number and licensee/permitee name.
Email is the method the people answering the phone will tell you. |

I sent an email through the Ask The Experts, and I received a reponse at 7:09AM this morning, saying that they will send a corrected copy. I hope it doesn't take too long to receive the corrected copy. |

I received a response from Gary from Ask The Experts at 7:07 am and received a new email with the correct form at 7:35am
Thanks Everyone! Now if my Form 3 will clear so I can get the Form 4 submitted |
