When sending in a form 1 on paper for a trust do you need to include all pages or just the first page? The rest of the pages have no information on them.

Form 1s need to be submitted in duplicate? I've only seen that on the form 4.

Look at the top of the Form 1 on the left hand side, right below the bold, black line. It says "Submit in duplicate"

Yea, you would think the form 1 and 4 would be a little more consistent.
