Barrel length would be a good start
ETA: Here's a Choate +3 on my 870 with a 21" barrel. Contact Max100 if you want a custom flush extension made (so I hear)

Here's a Wilson 2 shot extension on my 870P, This is an 18 inch barrel.
Just my .02, but I think a lot of the Wilson extension, it's build like a tank, better than the choates, and others I've used in the past.

I have one of Scattergun Tech's (Wilson Combat) two shot extensions on my 20 inch barrel 870, and think highly of it.

The Wilson Combat/Scattergun +2 should be flush on a 18" barrel since it is half an inch short on my 18.5". The Nordic and TacStar extensions will be slightly short of flush with an 18" barrel.
