I have an 11" model on my upper and after minimal use it is now loose.
It pivots to the left about 1/4" from dead center.
I've tightened the 2 set screws on the tab underneath, but it hasn't helped.
There's slightly under an 1/8 of an inch gap on that tab now and I know thats pushing it with this foregrip.
Any ideas on how to fix this problem?

Did you use the barrel nut that came with the hand guard?

The helicoils have more than likely come loose or are out of alignment.  Contact VTAC or Troy, they will take care of it.  If you want to fix it yourself, see if they'll tell you the helicoil size and you can do it yourself.  It's pretty easy to do.

Sounds like the barrel nut is loose because it is only moving to the left and you have tightened the bolts.  Thee forearm can't move to the right because of the index tab.  I would pull the forearm off and check the nut.

now that would be the suck, I'd check that for good measure
