How much should I crush the crush washer?

Until your flash hider is correctly timed. If you're not using one that requires specific timing, you dont' need a crush washer.

Tighten it finger tight, then turn at least 90° but no more than 460° until the timing is correct (the slots lined up correctly)

Timing - referring to having the flash hider turned to the point that the open ports face sideways and up, not down

Sorry to hijack, but is it possible to get a standard A2 suppressor and have it time correctly w/o a washer?

It's possible. you've got about a 1/360 chance that it will be able to time correctly. If you're willing to be off by 1° either way, you increase your odds to 1/120.

It all depends on how the threads match up. It's a lot easier to just use a crush or peel washer.

So the distance from the barrel to the ports on the hider does not matter as long as the ports are facing upwards and to the sides.

You ought not be screwing the thing so far onto the barrel that the barrel begins obstructing the ports... in fact I dont' even know if the threads go far enough to even allow that. But the portion of the flash hider w/out the port needs to be on the bottom. Reasoning is that when shooting prone it doesn't kick up dust.

the vietnam 3 prong and A-1 birdcage(slots all around) dont have to be timed
