Is there some kind of standardized form that the instructors are supposed to you to sign off on course completion to certify you for your CCW?

There is no standardized form for the proof of qualification. What is required as written proof of qualification is the instructor's name and his certification, make, model, caliber, and serial number of the handgun qualified with. My qualification certificate also had the score listed and 80% was the minimum required at the range I qualified at. I was monitored on my familiarity and safe handling of my firearm during the course of fire. Also reviewed use of force. Hope this helps.
