I’d say he has two options: send it back to Glock for inspection, or just get an aftermarket barrel.

Like the guy above me, I’ve never heard of this in a Glock.

Does this happen with different types of ammo? Good quality brass case ammo?

Because sounds like a bad lot of ammo not something wrong with the gun to me.

I can't think of a reloading process that touches only the first 1/3rd of the round.

Sizing will size at least 3/4 of the case. Expanding will only affect maybe 5% of the case right at the case mouth. Seating most bullets will have them sitting about halfway down the case. Crimping is just to remove the expanding enough so they'll feed, so also only that last bit around the case mouth.

Does the barrel chamber have anything wrong with it, visually? Roughly machined chamber?

I've reloaded thousands of 9mm and have never had an issue like that. Never really had any issue once you know how to set your dies in conjunction with your tightest pistol barrel.

DO not shoot the pistol anymore.  This is a chamber issue.  There is a problem with the chamber in the area where the casing is being left behind.  I had this issue with a Ruger Mini-14 years ago.  It would start after the barrel got hot.  The first 1/3 of the brass would shear off and remain in the chamber area.

Ruger determined it was an out of spec chamber, and when heated, tolerances opened up even more causing the issue.  New barrel and problem was solved.  I haven't seen or heard of this happening in years.

Glock needs to get this pistol back and a new barrel will resolve the problem.

I bet the reloaded brass are from a different gun.  Glock chambers are tight compared to my Gen 3 S&W’s.   You need to screw the sizing die down further and test the sized brass in the chamber of a Glock barrel.   I had this experience trying to reload ammo that functioned fine in everything but Glocks.   Some brass would not size down far enough so I just trash that brass or make sure I don’t try to shoot it in Glocks.

What do the cases that fully extract look like?  Is there a raised area near the mouth of the case?

I'm very curious exactly what ammo/brass this was happening with... Not surprised Glock doesn't want to deal with it, though milling the slide has jack to do with the barrel. That's one reason I'm on the hunt for a 19.5 MOS
