A huge pain in the ass with very tight controls. There have been some "beta" testers flying that are in very good condition overall, required to test every two hours, and maintain their blood glucose on the high side during the flight.
I have had two special issuance second class medicals before diabetes. For an illustration about how the FAA works, the first certificate showed up good for 30 more days. The issue was blood pressure and a rare skin condition that no doc except a dermatopathologist and some dermatologists have any clue about, damn certain no FAA doc. One of my partners wrecked our airplane about the same time I was dealing with the medical and trying to sort out the wreck for the corporation, and I had to take a break from flying. I suppose stress at work didn't help.
I don't think the guy that wrecked the airplane has flown since, although he is still active in our EAA chapter.
I am not completely against flying without a medical. Not against it at all. My weight and blood pressure are no issue now, skin never was.
