My employer is tacking on an additional $100/month surcharge for unvaccinated employees  to our insurance premiums. Is this legal?
Already have a $250 surcharge for keeping a spouse on employee plan, if spouse is eligible for their own coverage at their place of employment.

Insurance is a benefit so yes they can do it. They do it for tobacco users today. I have worked for companies that test for nicotine when they test for other substances.

I don't know about legality but that would be enough to look for a new job for me.

Yes, employers can offer discounts or impose surcharges on insurance based on vaccine status: https://www.benefitslawadvisor.com/2021/10/articles/health-premium-surcharge/employer-vaccine-surcharges-yes-you-can/
