That actually sounds like a modem issue.
If the wifi card connects to the router, the card should be good.

Look into whether your modem's recommended setting are compatible with your internet service.

It may sound weird, but I had to stop using my SB8200 modem, because Sparklight/CableOne won't tune up their signal to Motorola's recommended spec, and my modem would cut out constantly.

If it is an ISP provided modem/router, ask them for another one.

its possible the card is dying. Before yanking it, make sure you have the latest drivers installed for it.  Wireless cards can be VERY sensitive to out of date drivers.

If you decide to replace the card, this is my pick.

Had very very similar issue with wifi card in my thinkpad.  I had to uninstall it from device manager and then reboot to get it to work.  Machine would find card on reboot and it would work for a few days and then fail again.  FWIW I got a new card and have not had issues.
