I just fixed mine 2 weeks ago with some rubberized type stuff.
It's self leveling.
Can't remember the name but it was in the concrete dept at the hardware store.

Sikaflex is what you likely used. It's a very elastic caulk.

You could do a tile waterproofing like schluter ditra or a liquid one like red guard. Then tile over the patio afterwards.

Go grab a V Shaped diamond blade for your grinder  . Chase all the cracks to open them up slightly,  then fill with Limestone colored SL -1 .

Polyurethane caulk really works better.
It remains elastic enough to stick to the edges and stay in place.
