That is a shame  When I was with D.O.C. in Jersey, I was issued a Smith 4046.  I liked the weapon, it had a smooth pull and I liked the weight of it.  Now all of N.J.D.O.C. has H&K USP fullsizes in .40

From the pictures I've seen, it looks like the bullet is leaving the cylinder while the cylinder is out of time, subsequently smashing into the face of the barrel / forcing cone.   Look at the splash marks, especially of the one that kaboomed on the 4th shot ever fired out of it (note fouling on the cylinder face.)  

I had a 342Ti 38 Spl. that literally fell apart in my hands after less than 1,000 rounds.  It was my pocket piece.  I sent it back with a nice letter.  Three days later I had a brand new gun with my old serial number etched upon it.  No explanation given, no apologies conveyed...just a new gun.  I've fired the new one...exactly five times.  The only reason it's still in the stable is because I can't find a sucker.  

The state of manufacturing in the U.S. is alarmingly dismal.

My guess is that the barrels are failing from stress corrosion cracking from being over torqued, and aggravated by using a steel, maybe 17-7Ph, that is not forgiving for that mode.  A high preload stress from torquing the barrel down tight, pounding from shooting, and presence of some solvents could cause this failure.  I don't remember the maximum preload stress for 17-7Ph, but it's about 70 ksi for 17-4Ph, which is a much more forgiving alloy for this mode.  The particular heat treat is also a factor.

Are the carbon steel guns failing, or just the stainless?

No, a semi isn't necessary for everything, but a higher capacity weapon for a prison seems like a damned good idea.

I,m a corrections officer in  NC  and yes I first hand observed a barrle fly off during training . we shoot a .38+p  well within tolerances of a .357 frame   the weapons are Old.  I have shot the same revolver for over 10 years. we are  finaly we are getting replacements . in other news  S&W, Glock, and the state are currently involved in a 3 way court battle over  life time contracts , officer safety , and replacement costs.
latest news : The S&W M&P in .40 cal  will replace the wheel locks.

transtion period to begin in 4-6months
opinon of S&W auto loaders

The only time revolvers are issued are on outside escorts, road patrols ect. In a  SHTF scenario out come the AR15's, MP5's and 870's. In one demostration with the 870 pump, a CO fires it while another chucks 00 Buck in the loading port.......you can lay down a staggaring amount of firepower

So much for that old saying "revolver reliability"



I don't know anything about barrels breaking off.  That's bizarre.
But I'll bet the misfires are due to nothing more serious than the strain screw loosening up.
It has happened with my 686 and I've seen it happen with other people's 686s and it is a problem that can happen with any revolver that uses a leaf spring to power the hammer.  

Tighten the screw and use a little locktite.  Revolver fixed; no biggie.
