You place the baby somewhere in the cake.  Whoever gets the piece with the baby buys the next King Cake.  I've seen some people use an uncooked red bean in place of a baby, though.

BTW, a "New Years Pretzel" is a Danish shaped like a pretzel, hence the sugar frosting in the photo
