As others have mentioned, the S&S M-AX is an option, a tad pricey, but comes with "cool-points"
I also use a cheap Inforce Gen. 1 as a helmet spot--won't really function as well as a hand-held, but is an option as well.
That being said, I've found that I like having both a low-powered nav-light light one of the Princeton Tec models:
Princeton Tec Switch MPLS
Princeton Tec Charge PRO
...as well as a higher powered spot.
To be honest, I've never been a huge fan of the onboard illuminators on devices like the PVS-14--I always found them to be too bright for close-range nav, but too weak for any useful distance.
The small Princeton Tec nav/work lights are much better, and you get less of the "blinding flash" in heavy brush (I used to tromp around the backwoods of West Virginia pretty often with 'em, where it can get pretty thick and pretty damned dark), and the flex head allows you adjust the angle to give you useful illumination without the viewing-direction-only blast of the onboard illuminator.
