So I'm in the process of completing the responsible person questionnaire (dated 8/31/2022) that I downloaded from the ATF website and question 4b asks for the name and address of maker, manufacturer and/or importer of firearm.  In my case I'm completing the form 1 for an SBR from a B&T pistol.  This box used to have the caveat of "original maunfacturer" on it, but that no longer appears to be the case.  

So.....are they asking for my information as maker/manufactuerer or B&T, HK, etc in this question?

Yeah, they are asking about the original manufacturer or importer of the firearm.

I just copy the information from box 4a on the form 1.
