What would cause a misfeed like this?????

AA upper and piston, DD 14.5 barrel on DPMS lower

I have never seen a misfeed like this. has about 800 rnds through it over the past year.

today went through 3 mags sighting in open sights, red dot and an ACOG.

started some rapid fire and move and shoot drills.

This happened after a two mag drill, then changed out targets and went for round two...
first mag fine then half way through second mag this happened.

Dang thing was fed into the very top of upper and wedged above the piston rod...

I've only had that happen to me twice and I've been shooting ARs since the 70s. I assumed it was sorta like a doublefeed, and mag caused. (Not really sure exactly what happened though. And of course mine were wedged above the gas tube rather than the piston.)
I cleared it & changed mags & it didn't happen again. Mark that mag & see if you can make it happen again.

Last time it happened to me was in the 80s with Vietnam-era mags with black followers.

How did you remove that....did the bolt move at all...gass tube bent?

that's called a brass over bolt malfunction and is caused by a failure to eject ... check your ejector and your extractor maybe a chip of brass got caught in the claw or something in the ejector channel and to clear you Forcibly remove the magazine. Use the middle finger of the right hand to put force on the bolt through the magazine well while pulling back on the charging handle. After moving the bolt back a half inch or so, force the charging handle forward, allowing the casing to drop free.

Some thing caused your piston to not retract. Was the return spring broke or the bushing ceased up?

It was wedged in there pretty good but didn't take more than a few minutes to clear with a screwdriver to pull the round back.

the spring is not broken but I did notice that where it seats on the bushing that it is not coiled to a flat, just an open coil.
could that have let the bushing tilt and bind up?

It didn't seem to bend the rod at all, I rolled it on the work bench and did not see a wobble in it.

no that doesn't sound like that would have caused it and that rifle doesn't even sound that old with only about 800 rounds through it .did it have a o-ring around the spring and bushing ?if not you could put one if you start to have issues but it may  have just had some brass or the steel case ammo lube gumming it up or just a lack of lube one the bolt and bcg

Looks to me like a mag issue. Take the mag you used and load it half way. Now slam it down on a flat surface, on the bottom of the mag and see if any rounds jump out. If they do, throw it away, clear the jam, and it probably won't happen again.

PS: here is a misfeed very similar:

Thanks for the insight guys...

I didn't happen to save the mag but I know it's one of my OD ones so that breaks it down to 50/50 for me...

they are all magpuls tho, no cheap surplus ones so I hope its just isolated to one and not the other 16 to follow.

This is a mag issue.  Check your OD green Pmags for a crack at the back seam.  If cracked, this magazine just became a range only practice unit or parts fodder for repairing another Pmag.

That is a DI gun the gas tube does not move out of the way like the piston does on piston operating system. Something caused the operation rod to not move out of the way evan if it was a magazine problem the operating rod should had retracted. Worn return spring creeping up the operating rod, or a bushing binding.

Bolt override
Not related to operating system
Read green eyes black rifles
Or go to a legit training course.

And stomp that mag n put it in the dumpster

Good read here, never seen this before, kind of stumped how this could happen.
