As, a dentist, this would not be hard to do.  Just run the drill through the top of the tooth.  

It would probably be pretty sensitive after though, and you would end up with quite the cavity in a few months.

I would never personally do it though, no matter how much someone begged or offered to pay.  I do know a dentist or two however, that probably would take money to do this.

Seems like the only way to keep a cavity from occurring would be to pull the tooth, put in an implant, then drill that out for the piercing. Either way, pretty dumb in my opinion.

I suppose you could, but then it would be a pain to get the ring in and out.  The other issue would also be trying to bite down. Your lower teeth would be hitting on it.  It would likely just snap off the top of the tooth the ring is in, or chip the lower tooth.  Even an implant with crown would run into that issue.

Generally speaking, when you're getting shot at you want to make yourself small...added benefit of a more stable platform.
If you're not getting shot at, but there are plenty of targets, generally remaining prone is better.

You could always build up a pierced crown if someone's existing remnant needs reconstruction anyway. First give them a tongue piercing. After they crack their front teeth with that, you'll have a blank canvas.
