when a round clogs the barrel,and if another round is fired,could cause badness.How often does this happen??!??!?
and why? exactly what was wrong with the weapon/ammo?

If it happens once that is too many times. It is caused by a partial powder charge or no powder all. I've had 3 squib loads and I only had myself to blame as I reloaded those rounds. It will not cause any damage if you have the sense to unload immediately and clear the barrel.
