It seems everyone is doing c.q.b. Type courses. Does anybody teach courses on rural type operations? Everything I have ever trained for as a Police Officer has been in a urban environment.

Not sure exactly what type of training you're looking for, but ALERRT does rural terrain LE training.

Here are 3:
MVT Max Velocity Training: Good SUT from a former Brit soldier and contractor.  Based in WVA and Wa state.  Good book series, both training and fiction.  You sometimes have to translate the Brit terms for US but overall very good.  
John Mosby aka "Mountain Guerilla":  Good SUT from a former Ranger/SF dude,  Based in Idaho/ Western Redoubt Area.  Also has excellent book series: The Reluctant Partisan, vol 1 especially good.    
Mason-Dixon Tactical.  Good SUT from another former soldier.  Based in VA area.

ALERRT's Exterior Response to Active Shooter Events (ERASE) sounds like what you are looking for. Its an excellent course.
