Am I looking at the latest revision?

1 Name and Address of Registered Owner (full name, number, street, city, state and zip code)

I'm registered under a trust.  Can someone confirm that I should put the trust name and address here?

Any chance I can email them instead of snail mail?  [email protected]

The registered owner is the name/entity on the F1/F4. Sounds like that's a trust in your situation.

Yea, you can email them to that address.

BW is right. You put the registered owner's name on the form. I have a trust and that's the name I use.
