I did a little surfing on the battlerifles site but didn't see the pic. How'd that happen? Who'd it happen to? Are they grievously injured?

It looks more like a peirced primer than anything else, considering the hole in the primer.  If the cam pin wasnt in place, the brass wouldnt be in the chamber, it would have been pushed back by the force of the round going off.

I don't see what could have blown the rifle in half yet left the fired case more or less intact in the chamber.  Could the barrel have been clogged and ALL the gas came back down the gas tube?  Of course, the extractor probably would have yanked that case out, unless he didn't have an extractor installed??.  This one's got me stumped.

Oh wait, it's one of those .50 BMG uppers.

It appears that this upper failed as a result of very high pressure gas venting directly into the upper receiver rather than through the gas tube, into the bolt carrier and venting out the ejection port.  From the appearance of the gas tube, it appears that the gas entered the upper with the bolt carrier forward.  From the toolmarks slightly aft of the barrel extension, it would appear that the bolt remained lodged in the barrel extension and was probably removed after the failure.  The brass color of the primer tends to suggest a factory cartridge rather than a reload.  

Whether the failure was due to excess headspace, a bad reload as Troy mentioned (wasn't that Winchester 231?), firing 5.56mm ammunition in a .223 Remington chamber (I just had to throw that one in ) or some other problem you can't tell from one photo.

Some of the new primmers like the ww are brass color so it still may be a relaod...very dirty rifle even befor failure. See the heavy carbon build up all over the gas tube and upper?

Ummm...Did the top round in the magazine go off?  I'm having trouble understanding what could have done that much damage and left a round in the chamber.

Thank god, the only times I ever shot a squib was in a revolver.

I previously told on this board of the 2nd time I got a squib, which I reconized immediately.

Well, the first time, I aimed the revolver at the 7 Yard target. I pulled the trigger and pop. I see gas come out so everything looks fine. I then look at my target and for some reason have missed a target at 7 Yards. I figure either this gun is terribly innacurate or I've just become a pitiful shot overnight. So, I cock back the hammer for another shot. I pull the trigger and hot gas pours out of the cylinder and engulfs my gun hand. It burned a little, but didn't hurt me. This time, the cylinder won't turn anymore. So, I break it down and discover a bullet stuck halfway in the barrel and slightly out that is blocking the cylinder from turning. I remove that bullet and look down the barrel but can't see through it. So, I take a .38 Cleaning Rod and carefully place it in the barrel to discover an obstruction, my first shot. I hadn't misssed after all. I removed that bullet later on after I got home. I figure the 2nd bullet was pushed far enough that it allowed the gas to safely exit from between the cylinder and barrel.

Looks like full auto with a case rupture and subsequent brittle bolt fracture. I bet only a small segment of the case is intact.

Must have been a squib load causing a bullet lodged in the bore. Operator error. The great thing about the AR design is a load powerful enough to cycle the action will almost always kick the bullet out of the muzzle.

OK...split the bolt carrier but didn't mess up the charging handle (that's strange), bolt and firing pin are still there (no retainer pin), mag spring got done and the follower's missing but the mag's still in the well, and what does "Avertissement" mean?  Where did this occur?

Hey Big_Bear!  No fair!  I put a winkin' smilie on that one!

As for the failure,  Looking at pictures of the other two, it looks like the bolt carrier may have split at the camming channel.  If so, I bet the cam pin was stuck in the bolt.  I looked at the pic using a photo editor and it appears that the primer is severly flattened and possibly pierced.  There is also case head damage and what looks likes bits-o-brass in the barrel extension.

The carbon and soot buildup could easily have come mostly from the last round fired.

So what exactly is the issue here?  
By the looks of it, the bbl is ok.  Have it magnafluxed, screw it to another upper, check headspace and carry on.
