New set of Form 4s are going out under a trust.  The trust has only myself as the responsible party.  

My dealer is stating that since I am the only person on the trust, we don't need a 5320.23 and will just fill out the Transferee questions section on the Form 4.  

Is that a pretty standard approach?

This was my concern as well.  It states pretty clearly that each responsible person is required to submit a 5320.23.  They state that they haven't received any post 41F submissions back...I wonder if they just haven't realized that they are going about this erroneously.
