Whatever is engraved for both.
What you have for the model sounds correct, but I don't have one to confirm the manufacturer/importer. |

From my Form 1, circa 2015:
Importer I listed was: Ceska Zbrojovka, Czech Republic Official model I listed was: CZ Scorpion EVO 3 S1 This was when it available electronically. But, it was approved, so you should be G2G. |

My April 2015 eForm was approved with just SCORPION EVO 3 S1 as the model (no CZ)..
But it seems later approvals have been with CZ in the model name. |

^ correct. They accepted my as "Scorpion" but under the approved with following conditions my examiner wrote "CZ Scorpion EVO 3 S1". After about that time, some were being kicked back without the "CZ" , and E-file had "CZ"

Can anyone post an example of a form 1 for a scorpion for an individual. I am tring to go through the processes and only want to have to do it once but can't find to much about it unless I make a trust.

Anything specific to the firearm would be identical for individual vs entity. |

It's the registered owner (from the Form 1), plus the city & state where the firearm was actually made. Moving isn't an issue as far as the trust and engraving is concerned...but the USPS generally won't forward the approved form from the ATF if you move while it's pending. |
