Archived [ARCHIVED THREAD] - HELP with 41F question...tired of looking for an answer

Ok, I am filing out a form 1 (trust)  for an SBR.  On 41F - question 15.(Number of Responsible Persons ) ...do I just put my name???  Also, I have 1 beneficiary listed on my trust.  Do I put their name on #15 as well?....., and also, do I have to get background check/cards/photo for them (the beneficiary)?

I have literally been looking for an answer to this for 2 hours.  sorry if I missed something.

In most cases, beneficiaries are not responsible persons. Big Waylon put together a good tutorial sticky at the top of this forum page. Click here
