Archived [ARCHIVED THREAD] - Form 4 with trust- checklist

Sending my first F4 with a trust. Confirming:
No fingerprints
No picture
No compliance/certification

So all I need is:
2 form 4's, front side is normal, back side filled out throughout section 15, stopping at the CLEO sign off.
200.00 check
One printed copy of trust

Anything else I'm missing?

Back side of the Form 4 isn't necessary at all for suppressors or AOWs.  For anything else, all you need is Block 15 filled out.

Make sure you include all schedules and exhibits that the trust document refers to.
