I had my stuff ready to go and while double checking everything I found this post. I have the July 2004 formed filled out. Since ATF came out with the September 2007 form will the July 2004 get kicked back? Also, I had a State Judge sign mine. Is he supposed to sign as the CLEO or as a delegate?
Should I roll the dice and send the July 2004 form since it's ready to go or go through the hassle of getting another one signed? Th approval signature on the form I have is on the back. |

As long as the Judge is able to hear felony cases he qualifies as a signor. |

anybody know for sure whether or not theyre accepting the 04 form 1s offa titleii.com or not as i just sent 400 bucks worth that i printed bout a week ago?????

There is a sticky that says Atlanta now, WTF? I have read through 15 pages of B.S. trying to figure out where the hell to send this crap. Can someone clarify, Atlanta, WV, Bumfuck Egypt? C'mon this is insane.

If your form does not list WV for a Form 1 and GA for a Form 4, you're gonna be disappointed as the other offices have closed. Kharn |

Hey guys, I'm having a lot of trouble filling out form 1
I'm trying to set the rifle to be registered to my Trust instead of myself, Its my understanding that I don't need a photo or fingerprints, or a CLEO to sign off - Do I just leave all those fields blank on form 1? Could someone PLEASE explain this? I'm begging here, I need some help...ugh |

ATF Form 1 Rev Sept 2007 Here is a native PDF file of ATF Form1 - Rev Sept 2007. I painstakingly recreated it in Adobe Illustrator using a scan of the original as an underlay and then output a PDF. The PDFs on the web now are scanned images of the form, not native. Doesn't matter too much except to obsessive guys like me who want everything to look perfect. |

Last year I used the titleii.com html form with no problem, got my stamp. I am now working on a new SBR and seeing all this confusion around the Form 1.
I decided to call the ATF directly ((304) 616-4500) and ask one of their agents what the deal was. They told me as long as the revision is 5320.1 July 2004, the form is fine. She was familiar with the titleii.com website and said those forms are fine. She also said the new forms will work. She did say to print double-sided. So fuck it, I'm going to use that old one because I know it works. Will let you know what happens in about 2 months.. |

Here is the file that JohnStark initially made in Adobe Illustrator, but I edited it in Acrobat Pro to make it fillable. Probably the best looking fillable Form 1(latest and greatest BATF revision) so far.
Fillable Form 1 If you are having trouble with this file, go to Adobe's web site and download the latest version of Acrobat Reader. If someone can tell me what ALL of the acceptable options for 4b (Type of Firearm) are, I can update the form to make that a drop-down box. I just mailed off my Form 1 with "Short Bbl Rifle" in that box. What else do people type in there? |
