OK everyone, I'm new to the form 1 process so please be patient. I really want to get this right the first time. I have some questions:
1) I, myself, am going to be the manufacturer. Do I put my name in 3a as well as 3b? 2) On question 4h (Additional Description) do I need to put anything? There is nothing else on my CMMG, Inc receivver that I think I need to include. 3) After question 13 (which will be the CLEO's stuff to fill out) I listed my name on the line for Name of Maker. After that, do I leave all the CLEO info blank (for them to complete), like CLEO title, address, etc.? |

Has anyone been denied for using the rear signature block version? I just realized I'd used the July '04 rear signature block form, and I already got the CLEO signature (which takes 6 weeks and costs $23 in Maryland).

I just recently had my Form 1 approved. It had the signature block on the back. I think the ATF is taking all versions of the Form 1 until they get the official version posted on their website. |

My examiner returned my message after I left work yesterday, the rear signature July 04 form is still acceptable but she did not give any information on if/when it would cease to be acceptable. I'd suggest using the Sept 07 form if you've got a long CLEO process, just in case.

This is the one we should be using (Dated Sept 2007)? It shows WV address, is that the current address it needs to be mailed to? Overall Length should be "Open/Extended Stock Position" right? |

I just sent a form 1 today and spoke to NFA about the address. Send it to the WV address. There seems to be a lot of confusion on this so hopefully this clears it up.

I have my forms filled out for my SBR application and will try and get a signature tomorrow from my county Sherrif when I turn in my concealed carry application. My question is this....I currently have an offer in on a house and my address will more than likely be changing(same town) before my forms are approved and sent back from the ATF. Do I just need to contact their office and let them know that an item on the application has changed. Or do I need to submit a formal letter/document/form informing them of the change, even though an approval hasn't necessarily been granted? I hope my question is clear. Thanks for your help in advance.

Just to capsulize things, if I'm using my trust to make an SBR, then I can completely ignore the stuff on the back of the form about the photograph and the Law Enforcement Certification, right?
I send this in in duplicate (with a Certificate of Compliance in duplicate-I like both a belt and suspenders with touchy subjects) with my $200 check to the address on the Sep 2007 form, right? |
