The mag falls out when the gun fires about 75% of the time. I am not pressing the mag release button. I shoot it one handed naking sure I am not touching the mag button and when the gun fires, the mag falls about 2 inches out of the grip and the next round is not chambered.

The button does seem very easy to operate.

Any idea what might be wrong with it.

I took out the magazine safety bar and replaced that spring and also installed the APEX Enhanced carry and duty kit, but I don't see how that has anything to do with mag release. Somehow the recoil is dropping the mag.

Is the magazine catch spring properly engaging the magazine catch?  

Does the magazine catch have a little metal insert?  This is the part the actually engages the notch in the magazine body?

How old is your M&P? Older M&Ps have this problem. Call up S&W and they will take care of you.

It is locked up solid until it fires.

Thanks, after some googling, I found that it is a known problem with early M&P .49s and I will be contacting S&W for the redesigned mag catch.
