hmmm yesssss. I will be needing to fill this out soon as well. Been too busy with clinicals to get it done, but hopefully with it calming down the next week or two, I will get them sent out. All I have gotten done so far is gotten the last of my mags that I need, ordered all of the parts, pulled and replaced the fsb to a threaded one, and now just a waiting game for the form to come back. That is once I get it mailed.

Arsenal of Las Vegas it is! My Form 1 is ready for my CLEOs signature. I'll have him print me as well. It's nice living in a small town sometimes.

I put "Arsenal Bulgaria" then Arsenals info below it. It was what was on the gun. Mine got approved no sweat!
Just remember you may or may not have to open the Gas port on it. Mine needed some TLC but runs like a champ! Tread |

My Form 1 is currently pending approval, so I hope I was right when I put the info for Arsenal Las Vegas in that spot.

I called Arsenal to double check and they said that the Las Vegas address would be the correct one.
