disconnector spring improperly installed or failing.
ETA - most likely culprit but check to make sure disconnector and fcg are all as they should be and don't have any abnormalities. Perform function check. |

Disconnector hook not grabbing hammer, spring wrong or disconnector hook worn
Trigger pin walking out of receiver. Makes the nose of the trigger misallign with hammer, does not reset and allows hammer to fall. Check the hammer spring installed so thae legs rest on the trigger pin groove, also that the spring is not installed backwards. |

Check for broken hammer pin.
