Definitely Gear Sector. I've been using theirs for a couple years on my M4 at work. Rock solid and comfortable round profile that lines up with my Magpul rail covers.

Depends on where it's going.
I like Fortis RAP for top rail mount (90* hole), and Magpul for side rail mount (45* hole). I don't like straight out side mounts as they interfere with my hand placement options.
There are MANY designs/makes so take some time to check them all out and think what would fit your needs best. There's no shortage of choices!
