14.5" middy, VooDoo low mass carrier, H2 buffer and standard carbine spring.

Have standard carbine buffer and standard M16 carrier in the box.

Lighten to H weight buffer or to standard carbine?

Happens with OK ind, pmag gen 2,3 and lancers using 855, 193 and wolf.

Put the standard buffer in and use commercial brass ammo to limit the potential problems. Mix and match until you experience the issue again.

It could be any combination of parts, as too heavy or too light, too much or too little can all cause this.

Make sure your gas block is aligned properly. If that problem isn't alleviated, change to a standard weight for the buffer. If still having problems, look at the bolt catch or try a reduced power spring.

First post so I may butcher this...

But as everyone else has said try:

1: standard buffer
2: use the m16 BCG you have (If you have a NIB it may have less friction and help cycling)
3: check gas block alignment

*2: I just built a dissipator (16 rifle gas barrel) and used the same low mass carrier for it hoping I would be able to adjust the gas down for a freakishly soft shooter... I had the syrac gen 2 block wide open and it wouldnt lock back. I threw in a spare m16 NIBx carrier and was able to lock back and function on  a full mag on setting 10 (18 being wide open).  Also had a buddy with the same low mass carrier and he had similar issues until we swapped out to a regular BCG.
