Hello all my Trust has been through the system and I got an approved form 4 and have my SBR.

Now that I have it back I am going through to update the Schedule A. I notice that I have a Schedule A and and Assignment of Property, one page for each. They say pretty much the same thing so can the just be combined? I looked at a few other sample trusts and on theirs its all in one. Its Titled Schedule A and then the next line is Assignment of Property. So can I just do that to mine? I can provide screenshots if necessary.

The other thing is in a couple spots it  says transfer to ME trustee of X trust dated ______. Executed at _____, Date _____. Is the first dated the date the trust was created and if so do I need something at the very beginning like X trust effective X date?

In general, X trust dated Y refers to the date Y on which the trust was most recently amended at the time you were signing.  If it has never been amended, it's the date on which the trust was created.  If it has been amended most recently as of new date Z at the time you are signing for something, then you refer to it as X trust dated Z for that particular signing.  

I don't know how or if specific trust law in your state may require dating in the body of the text or not - that's properly a question for a lawyer licensed to practice law in your state.
