I was browsing the 1911 forum and I saw this post. A Sprinfield 1911 cracked slide using standard ammo. Someone there refers to another incident so I guess it's not unique. Very scary situation, since there is no slide stop on 1911's.

I think he is referring to something that Beretta made famous.  After the incident with the SEAL's, Beretta put a device in their gun to prevent the slide from flying back and hitting someone in the face, should it begin to crack, due to improper use of hot loads.
