Which series gun?  I've been told that the Series 70 design can supposedly have the firing pin travel forward with the safety on which is why they made the Series 80.  Seems like that, plus a soft primer, could have caused your ND.

Holy shit!!! I'm no expert but it certainly sounds like some sort of slam fire (wrong terminology) but a discharge caused by firing pin inertia from the fall. I hope everything goes ok from an employment perspective.

That safety looks like it is popped out of the frame (and for that reason not engaged).

I would count this as an AD.

Mechanical failure of the fire arm under normal conditions.

This is the only thing I can think of.
Does the pin rest on or close to the primer?
Does the safety only block the trigger pull?

No, I don't own a 1911. Never have.

many people recognized it as a gun shot, many didn't, and some didn't hear it at all.

some ran for the doors, others ran around (the two owners) to make sure everyone was ok, and most just sat in their desks without doing anything.

the owners no I carry, and they actually were yelling for me and came looking for me, so that kinda made me feel good...they thought someone was shooting the place up and thought I would be there to help.

when the thing went off I just sat there and thought "did that really happen"?

little pieces of shrapnel are all over the bathroom, I have some little cuts on my right arm.

It went off because you took it out of the holster and had it pointed at something you did not want to destroy.  I carry concealed on a daily basis.  Yes, it complicates dropping a deuce, but rules are rules.  Leave the damned gun in its holster when you take a dump.  Good on you for learning this painful lesson.  Glad no one was (seriously) hurt.

So if I'm looking at this correct, since the slide lock isn't engaged, the friction from the safety is holding the slide back?

You might want to later try:

Unload
Disassemble
Put safety back into frame properly
Reassemble
Get a hard plastic tip hammer
Cock the hammer back, engage safety.
Whack the nub of the safety on the opposite side of the frame.

See if the hammer drops.

I have done this countless times, and I even remember the first time I thought about putting it there...I was thinking how clever I was that I found a new "secure" place to put my gun when taking care of business.

So today I go in for the routine....

I take gun out of holster, place in ass gasket holster, and this time the gun stays for a second and then drops right on through the bottom and lands muzzle first on the tile floor....I remember watching it start to fall thinking for a split second that I hope it doesn't ding my gun.....

THEN BOOM!!!!!!!!!!

This is the part the I DO NOT UNDERSTAND.  

How does the gun go off with the grip safety not engaged and the thumb safety on?  You can see in the picks the thumb safety is STILL ON!! IT IS JAMMED BACK?????

Anyway, call me stupid, whatever.....but this is what happened.

ANY IDEAS ON HOW THE GUN COULD HAVE GONE OFF WITH THE GRIP SAFETY NOT ENGAGED AND THE THUMB SAFETY ON?
the thumb safety is still on!

This guy did a series of drop tests and dropped a series 70 1911 from 15 feet on the muzzle and was unable to get the gun to fire. If your firing pin spring is worn out or weak, that can change the results drastically I would imagine.
