I once worked at a summer day camp as a counsellor. The first thing they taught us was to never, ever let the kids out of sight. If you weren't walking around the playground keeping an eye on things you weren't doing your job. My guess is that the counsellors weren't being as attentive as they said they were and are all covering for each other.
As for the kid, it's probably abuse or something they've seen from their parents or older siblings.
