I'll guess:
The instructor added power and the plane began to climb.
The instructor completely forgot what he was doing and started to explain the situation to the student.
The instructor reduced power when he did that. Possibly this was habit from practicing stall recovery with students at altitude and he forgot where he was.
The student was not fit for the left seat and didn't correct the mistake. The student didn't understand that lift is more important than instruction.
The instructor realized his mistake and jerked the stick all the way back. This could be panic or it could be because he believed that belly into the trees was better than nose into the trees.
