From what I gather, it was actually pretty painless.  He was out cold (no pun intended) for the bulk of the flight.

The B767 had a pretty tight seal on the main gear compartments. It's still unpressurized, but it would be pretty warm for the first few hours of the flight. Young kid, in good shape could survive the high altitude for several hours and they have before.

Biggest surprise to me is how he was not crushed when the gear came up, or fell out on extension.
