I've only had to change one in a mustang I had but I had a ton of issues with that car. You really shouldn't need to be changing the pcv valve unless the car has a ton of miles on it and is having issues.

Modern engines have much less blow by....that leads to a cleaner PVC valve that doesn't need replaced as often. Some are a down right nightmare to get to.

When the ECM throws a code, sets off the Check Engine Light and after reading the code the PCV valve is the problem.

On my 98 accord DD beater I had to do it around 2013.

I noticed when decelerating down a hill while in gear as soon as I gave it gas again a puff of oil smoke would come out tailpipe.

Replaced pcv valve and problem solved.

In the past year or so, we've had a couple of jobs where oil leaks occurred after major repairs were made. One was a Subaru timing belt job where the cam seals had been replaced (not at our shop)  but, started leaking soon after the customer got the car back. We replaced the seals and backtracked the problem to a plugged PCV valve.  Now, any time we do cam seals, we do the PCV valve as a part of the job. $20.00 worth of cheap insurance against a comeback.

I changed them out on my FJ Cruiser and my wife's 4Runner at 90k miles.  Mainly because the parts were cheap, easy to replace, and I was in the mood to do some preventative maintenance.

I replace the one on my jeep every year or so. Cheap insurance against blowing out gaskets.

I do mine at 100K on the newer rigs, on my older stuff I change em ever 30K.

I check mine at each oil change.
As long as it still rattles, I continue with it(granted, my newest vehicle is a 2002, the oldest an '87).

Replace the grommet also. Caused a myriad of problems with my 22re powered 91 Toyota pickup with 207k. Mirrored a MAF sensor failure in a Ford EECV system.

New grommet/valve/hose is a night and day difference.

*Edit* valve still rattled but did not function as a check valve even after a liberal cleaning with brake clean. I could suck and blow through it. OEM replacement was dirt cheap and made me feel a lot better.
