If it is an employer sponsored plan I can most likely look it up if you know the employer.

Insured via Lloyds of London.....that's a serious player there.

Send a contact email with your info in it and that message to any email address of theirs and I'd bet you have contact by the end of the day.

Negative on that. They advised that they wouldn't know who issued the binder.

Assuming this is in Florida, report the situation to the Florida Dept of Financial Services/Division of Insurance Fraud. http://www.myfloridacfo.com/ They exist to deal with fraud. If there is merit, they will aggressively pursue it. You may be eligible for a reward, too.
